Job summary
The Humbleyard Practice has an opportunity for an experienced nurse or ECP to join our nursing team. This is an exciting role comprising a professionally rewarding mix of outreach visiting of housebound patients, care home support and in-practice clinics. We are a flexible employer and can work with you to agree a working pattern that is mutually acceptable.
Main duties of the job
You will manage a caseload of housebound patients for whom, with their registered GPs support and direction, will provide advice and assistance with their continuing medical needs. You will undertake acute visits on a daily basis and provide care for patients from initial history taking, clinical assessment and diagnosis, liaising with the patients GP, as required and the implementation of treatment plans. You will demonstrate safe, clinical decision-making and holistic care.

We work as one caring and supportive team, over three sites (Cringleford, Hethersett and Mulbarton) and pride ourselves on providing holistic, patient-centred care to our local population.

Qualifications
Essential
1. NMC or HCPC registration.
Experience

2. Have knowledge of the needs of patients with long-term conditions.
3. Possess excellent communication skills.
4. Be able work collaboratively with the practice team to meet the needs of patients, supporting the delivery of policy and procedures.
Desirable
5. Independent Prescribing qualification.
6. Post registration qualification in long term conditions management.
7. MSc in Health & Social Care.
