An exciting and rare opportunity for an experienced OccupationalTherapist to join our specialist team within St Luke's to help deliverour Strategy Every Death Matters with a key focus on earlyintervention and identification of palliative and end of life care.
Within St Luke's therapy team their key focus is on rehabilitation palliative care enabling people to live fully until they die.
Theywill be working predominantly within the specialist community teamproviding high standard of Occupational Therapy assessment with patientsin their own home alongside the Clinical Nurse Specialist team. The AHPteam offer an in-reach service to patients admitted to the inpatientUnit, assisting in discharge planning and therapeutic interventions.
Weare seeking an enthusiastic and highly motivated individual to join themulti-disciplinary team in the provision of specialist palliative careto patients and their families.
The post holder will need to beable to work autonomously and be responsible for complex holisticassessments and planning care to meet patients practical,psychological, spiritual, and social needs. They will work closely withprimary and secondary care professionals and Hospice colleagues, andgood communication and team working skills are essential.
