Job Overview
An exciting opportunity has arisen for an enthusiastic, dynamic, and motivated General Practitioner to join our Frailty Team working as an integral part of our multidisciplinary team. We are looking to increase permanent senior medical clinicians within our medicine and Frailty teams. We are keen to include GPwSI in our teams and improve the interfaces between primary and secondary care.
The successful candidate will provide senior review for patients suitable for admission to the frailty virtual ward providing hospital care at home, delivering a high standard of clinical care whilst supporting new ways of delivering quality services.
The ideal candidate will have knowledge of Care of the Elderly / Frailty, as well as an interest in general medicine or the motivation to learn and professionally develop while working in a supportive environment.
Main Duties of the Job
The posts are substantive posts and therefore essential criteria includes:
* Full GMC Registration
* Entry on the specialist register as a GP plus a special interest in care of the elderly
* Motivation to undertake further development and studies in this area
The role will ensure that frail older people in a ‘crisis situation’ receive expert, responsive, and safe care in a place closest to home, that best meets their needs.
Practice as an autonomous General Practitioner with special interest in Frailty, demonstrating advanced clinical competence and utilising highly specialised knowledge and skills to diagnose, plan, deliver and advise on programmes of care for patients.
The post holder will:
* Accurately assess a patient including history taking, examination, diagnosis, investigation, treatment, and where necessary referral onto multidisciplinary colleagues in a timely manner.
* Provide professional leadership, expert knowledge of frailty management, education, clinical advice, and experience to multidisciplinary team members.
* Maintain accurate clinical records.

Registration
* Full GMC Registration
* Valid Licence to practice with the GMC
* MRCGP or evidence of equivalent training and experience
* Entry onto the Specialist Register or a Specialist Registrar within 6 months of the award of their CCT
* Post Grad Degree
* To be formally recognised as Educational Supervisor as defined by the GMC Trainer Standards
* Diploma in Geriatric medicine or motivation to complete
Experience and Skills
* Wide experience in General Medicine
* Comprehensive clinical experience of Elderly Medicine
* Ability to communicate and liaise effectively with patients and their relatives and with other members of the MDT
* Ability to work and make patient management decisions on own initiative while seeking appropriate guidance from colleagues when required
* Ability to work as part of a team with professional colleagues, both medical and other disciplines
