The post holder will assist the registered dietitian in providing nutritional support to adult patients in the community setting. The majority of these patients will be elderly and frail and residing in a Care Home.
They will assess residents nutritional intake and implement a range of nutritional care plans (within protocols and scope of practice) monitor progression, and make alterations to the nutrition care plan as required.
The postholder will work as an autonomous practitioner liaising directly with resident family members, Care Home staff and other healthcare professionals optimising nutrition within this setting under the regular support of a registered dietitian.
The post holder will support the provision of Nutrition and Hydration training to care home staff.,
* Assist in the provision of high-quality nutritional care to patients within the community setting
* Monitor and evaluate patient progression, making alterations using agreed protocols under the regular supervision of a Dietitian
* Provide nutrition support to an agreed case load of patients as specified by the supervising dietitian working within their scope of practice-Undertake visits and assessments in a variety of settings as required by the need of the service including a patients home or other community setting
* Have an understanding of acute conditions long term conditions and common pathologies and the role of diet and nutrition in treating them
* Travel to Care Homes supported by Nimbuscare GP practices on a regular basis providing nutrition support and education to their residents and staff members
* Be responsible for planning and prioritising own day-to-day allocated workload and resources-To recognise variances in expected clinical outcomes and to act appropriately by escalating to and seeking guidance from a Dietitian
* Support registered staff and carers to identify malnutrition-Provide nutrition information and advice to patients and family carers verbally and in writing-Provide support and training to staff and students
* Contribute towards team audit and projects including the collection of relevant data to improve service provision-Keep complete clear accurate and timely records utilising digital platforms as required in accordance with department guidelines

We also aim to shift services into the community where possible, for example our community diagnostic centres run in partnership with the Acute Trust.
