Main duties of the job
We are looking for an experienced nurse, who acting within theirprofessional boundaries, will provide care for the presenting patient frominitial history taking, clinical assessment, diagnosis, treatment, andevaluation of their care.
We welcome applications from nurses currently working in either PrimaryCare with experience in management of long-term conditions including COPD,asthma, diabetes etc.
The post holder is anexperienced clinician, who acting within their professional boundaries, willprovide care for the presenting patient from initial history taking, clinicalassessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinicaldecision-making and expert care for patients within the general practice. They will work collaboratively with themulti-disciplinary general practice team to meet the needs of patients,supporting the delivery of policy and procedures, and providing clinical leadershipas required.

Job responsibilities
CORE TASKS AND FUNCTIONS
The post-holder will:
1. Make professionally autonomous decisions for which they are accountable.
2. Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.
3. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
4. As an independent prescriber: to prescribe safe, effective, and appropriate medication as defined by current legislative framework.
5. Provide safe, evidence-based, cost-effective, individualised patient care.
6. Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care.
7. Promote health and well-being using health promotion, health education, screening and therapeutic communication skills.
8. Refer patients directly to other services/agencies as appropriate.
9. Take the Practice lead in developing and delivering specialist primary care services, such as long term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease and hypertension)
10. Work with nursing, medical and health care assistant colleagues to ensure that National Service Frameworks ( Coronary Heart Disease/Older People/ Diabetes /Mental Health) are being delivered.
11. Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.
Participate in identification of community health needs and develop patient/family-centered strategies to address them.
Help develop and set up new patient services and participate in initiatives to improve existing patient services.
Other responsibilities with the Practice:
Awareness of and compliance with all relevant practice policies/guidelines, prescribing, confidentiality, data protection, health and safety, supporting annual Quality and Outcomes Framework (QOF)Assessment of the Practice.
A commitment to life-long learning and audit to ensure evidence-based best practice.
Contributing to evaluation/audit and clinical standard setting within the organisation.
Contributing to the development of computer-based patient records.
Attending training, internal meetings and events organised by the practice or other agencies, where appropriate.
Meet all tight timescales/deadlines to ensure that the Practice meets quality standards and receives the designated funding ( Quarterly Enhanced Services returns, annual QOF audit etc.)

Work with the Partners and Management Team to achieve standards of quality, performance standards, budgets and targets without compromising levels of patient healthcare.
Contribute towards the development and implementation of new standards, policies and procedures that are/will be required of GP Practices now and in the future.

13. Previous post registration experience
14. Evidence of appropriate continuing professional development in all aspects of the Nurse Practitioner role
15. Experience in management of long term conditions eg Asthma, COPD, Diabetes, CHS etc
16. Evidence of working autonomously and as part of a team
17. Proven ability to evaluate the safety and effectiveness of their own clinical practice
18. Understanding and knowledge of policy developments related to the delivery of Primary Care Services including General Practice, Clinical Governance, Quality and Outcomes Framework.
19. Full and valid driving licence and access to a car for work purposes is required
20. Current registration and appropriate representation with a recognised medical defence organisation
