You will also work with and triage other patients outside of frail and elderly. You will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. You will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. A large percentage of the role will be involved the care of frail and elderly patients including the responsibility of patients within care and nursing homes. This will include weekly ward rounds. Part of your responsibilities will include visiting patients in their home to assess their medical condition and frailty review. You may also be involved in triage of patients.
