Mid CRHT are currently looking to recruit Band 6 practitioners to join the team.
CRHT operates on a 24 hour, 7 day a week basis, providing intensive home support to service users as an alternative to hospital admission. The team works collaboratively with a variety of services to support service users at home.
CRHT is based in St Helens and will cover other surrounding boroughs: Halton, Warrington & Knowsley.
The post holder will be expected to work on a rotational shift basis in accordance with service demands, including weekends and night shifts.
Responsibilities include:
1. Assessment of care needs and the development, implementation, and evaluation of programmes of care within the framework of Effective Care Co-ordination (ECC).
2. Operating as part of an integrated Home Treatment Service and delivering care in various environments.
3. Assessing and providing treatment to service users with complex needs, considering alternative options to admission where possible.
4. Demonstrating an attitude that respects and values service users’ choices, working alongside them and their carers to ensure they are at the centre of care planning.
5. Participating in a 24hr roster system, 7 days a week, on a rotational basis as service demands.
6. Conducting face-to-face assessments, specialist mental health assessments, and acting in an advisory capacity in conjunction with mental health law.
7. Delivering training and education to police colleagues and signposting to appropriate partner agencies.
