A permanent vacancy has arisen for an Advanced Social Work Practitioner, to be based at Salford Older Adults Community Mental Health Team (CMHT).
This is a great opportunity to work within Salford, a diverse and vibrant city, within our integrated and multi-disciplinary community mental health services. The team is situated within Greater Manchester Mental Health NHS Foundation Trust (GMMH) where the mental health social work function is delivered in accordance with local integrated arrangements between Salford City Council, Northern Care Alliance and GMMH.
You will provide positive leadership and support to social work staff and ensure the delivery of effective services for adults experiencing a range of severe and enduring mental health conditions.
The post holder will lead on the delivery of statutory social care functions at the CMHT in accordance with the Care Act (2014), Mental Health Act (1983, as amended), Mental Capacity Act (2005) and other relevant legislation.
You will need to model effective practice in complex situations, providing a strengths-based and person-centred approach while managing identified risks. You will need to network with a wider range of professionals and other colleagues, promote a culture of professional curiosity and contribute to the positive social work identity within the organisation.
Key Responsibilities:
* Provide consultation and guidance with respect to complex social care practice issues to social workers, care coordinators and other staff within the team.
* Assist the Team Manager in providing effective social care leadership and ensuring high standards of practice.
* Support and provide supervision to social workers working in the CMHT.
* Carry out social work duties and interventions in relation to complex cases, requiring high levels of skill, knowledge and professional expertise. Act as CPA Care Coordinator for a small caseload in recognition of other duties.
