Job summary An exciting opportunity has arisen within Warrington Innovation Network to recruit a Senior Clinical Pharmacist to lead our PCN Pharmacy Hub team. We are looking for a dynamic and skilled Clinical Pharmacist who may already be established in a leadership role or who would be interested in developing their career in a leadership role. You will be joining a team of pharmacists and pharmacy technicians and be responsible for the leadership, management and training of the pharmacy team to ensure delivery of exceptional care for our patients. Main duties of the job The role of PCN Senior Advanced Clinical Pharmacist is a varied role, essential to the day-to-day operation of our PCN Pharmacy Hub. The role will be crucial in supporting the delivery of effective, safe and high quality of pharmacy services to the PCN population. The post holder will be the first line of support and seniority for all pharmacy team colleagues on site and will be responsible for a range of clinical and leadership duties. Undertake patient facing clinics independently within the scope of your prescribing competency Undertake medication reviews of PCN patients ensuring involvement of the residents, relatives, care home staff and GP practices and liaising with the GP, Community Pharmacist, and Care Home Manager regarding the outcomes.
