This role involves working with children and young people with disabilities and their families to ensure they receive the appropriate support and services.
Key Responsibilities:
1. Conduct assessments of children with disabilities and their families, identifying strengths, needs, and appropriate interventions.
2. Work closely with health professionals, education providers, and other services to ensure a multi-agency approach to support.
3. Provide advice, guidance, and advocacy to families, ensuring that their voices are heard, and their rights are respected.
4. Review and monitor care plans regularly, ensuring that interventions remain effective and are adapted to changing circumstances.
5. Ensure that all casework complies with relevant legislation, including safeguarding, and that the best interests of the child are prioritized.
6. Attend and contribute to multidisciplinary meetings, case reviews, and safeguarding discussions.
Requirements:
7. Qualified Social Worker with valid Social Work England registration.
8. Previous experience working with children with disabilities and their families is essential.
9. Strong knowledge of relevant legislation, including the Children Act, Care Act, and safeguarding procedures.
10. Commitment to promoting inclusion, equality, and the rights of children with disabilities.
