We are looking for an Advanced Nurse Practitioner to join our friendly experienced team at our Sparcells site. The role will involve working independently as an autonomous practitioner providing clinical assessment, diagnosis, and treatment for our adult and paediatric patients with undiagnosed and existing conditions. You will be working alongside our clinical team of GPs, Nurses, Pharmacists, Physiotherapists, Health Care Assistants, and other associated roles to provide safe, holistic care to our patients. This will include face-to-face and remote appointments. You will be required to work within your professional competence and boundaries to provide both urgent and planned care to patients.

Job responsibilities
Job Title: Advanced Nurse Practitioner
Grade: Dependent on experience
Reporting to: Nurse Manager
Job Summary/Main Purpose: The post holder will work autonomously to provide comprehensive healthcare that is sensitive to the needs of the population. They will take a leading role in managing the clinical services provided by the practice, ensuring that they meet the needs of our patients and fulfill the terms of our contracts.
SPECIFIC RESPONSIBILITIES
Clinical
1. See patients that present with previously undifferentiated, undiagnosed problems, taking responsibility for their health assessment and physical examination, the diagnosis of their condition and its management or referral to another member of the multi-disciplinary team deemed appropriate by the practitioner within their professional competencies.
2. Make differential diagnosis using decision-making and problem-solving skills.
3. Make professionally autonomous decisions for which they are accountable.
4. Order necessary investigations and provide treatment and care, both individually, as part of a team and through direct referral to other primary and secondary agencies.
5. Plan, implement, and evaluate the chronic disease management programmes in line with current evidence-based best practice and the GMS contract, reducing unnecessary hospital admissions and promoting the preventative aspect of nursing practice.
6. Prescribe appropriately as an Independent Nurse Prescriber, using the practice formulary.
7. Screen patients for disease risk factors and early signs of illness following NICE guidelines.
8. Create patient care plans to include health promotion, health protection, and disease prevention as appropriate.
9. Record information in patient records accurately and in line with current professional guidelines.
Management
1. Work to the NMC Code of Professional Conduct.
2. Provide leadership and support to the clinical team at the practice.
3. Actively monitor progress in the delivery of contracted services.
4. Make a significant contribution to the development and direction of the practice.
5. Participate in multi-disciplinary meetings.
6. Participate in staff appraisal.
7. Promote communication and provide learning opportunities for other staff.
Education, Training and Development
1. Hold a Masters degree in Advanced Practice.
2. Develop new skills by undertaking training as required.
3. Maintain current skills by study and attending update training.
4. Maintain a professional portfolio and actively keep up to date with the latest developments and guidelines for best practice and disseminate this information accordingly.
5. Take part in the appraisal process.
6. Revalidate in line with national guidelines.
7. Encourage, facilitate and participate in clinical supervision within the practice.
8. Be a resource for advice and guidance for other members of the team.
Research
1. Monitor research in order to improve quality care and disseminate research findings.
2. Ensure evidence-based practice personally and throughout the clinical team.
3. Undertake an active role in research and audit to better improve patient outcomes.
Person Specification
Experience
* One year experience post Nurse Practitioner Degree.
* Experience of multi-disciplinary working.
* Able to demonstrate high level of personal professional autonomy and accountability.
* Work using advanced clinical skills.
Clinical knowledge and skills
* Competent clinical skills with a knowledge of chronic disease management.
* Able to use analytical skills in practice.
* Excellent interpersonal and organization skills.
* IT experience.
* Clear understanding of legal issues, professional accountability, and responsibility.
* Experience of SystmOne.
Qualifications
* RGN.
* Recognized 3 year Nurse Practitioner Qualification to include autonomous practice.
* Chronic Disease Management - Asthma, Diabetes, Chronic Obstructive Pulmonary Disease, Coronary Heart Disease, Stroke, Family Planning.
Other experience
* Excellent written and verbal communication skills.
* Experience in liaising with General Practitioners and other Health Care Professionals.
* Experience in managing RGNs and Health Care Assistants.
* Ability to be flexible and adaptable to change.
* Understanding of clinical governance.
* Ability to work under pressure.
* Able to manage time effectively.
