Please apply for this job as soon as you can, if interested Birmingham Stroke and Neuro Service has an exciting opportunity for a Band 4 Rehabilitation Assistant to join our motivated and highly skilled multidisciplinary team. The Team are looking for someone who is comfortable working within a supportive team to assist and complete rehabilitation duties as prescribed by our Physiotherapists, Occupational Therapists and Speech and Language Therapists to patients with stroke and neurological conditions. The Team is responsible for providing assessment and treatment to patients with stroke and neurological conditions in their own homes and also rehabilitation to patients in care homes. There are numerous opportunities to work alongside other professionals within our partnering teams. Main duties of the job Work as part of a community multidisciplinary team. Carry out specific designated delegated treatment programmes and related tasks, evaluating these and adjusting within agreed parameters. They will report back, to the delegating therapist, any adjustments to the agreed programme. Monitor patient progress and report changes to the registered professional and follow changes made to that care plan. Assist therapists to monitor and provide treatment and rehabilitation, including providing appropriate pictorial and written exercise programmes/ communication books, as guided by the therapist. Carry out appropriate outcome measures (e.g., Berg balance, 10 metre walk) upon request of the therapist. Participate in regular, goal planning, review and discharge planning meetings with the therapists to review the patients under the post holder's care and agree an action plan To comply with the infection control processes and policies of BCHC. Respect the dignity, wishes and beliefs of patients and carers; involve them in shared decision making; and obtain patient consent prior to all interventions. Order, deliver and install equipment as directed by a registered professional. Communicate effectively both verbally and in writing with colleagues and stakeholders, utilising electronic methods of communication within Policy and Procedures set out by Information Governance. Plan and organise own workload taking into account the priorities set by the coordinating professional.
