Assistant Practitioner / Nurse Associate/ HCA
An exciting opportunity has arisen for a band 4 position in Integrated Community Teams South in Waltham Forest.
We are seeking a motivated and dynamicBand 4 assistant practitioner/nursing associate, HCA ,with an interest in general physical health for this exciting opportunity to work within the Integrated Care team in Waltham Forest. Integrated Care Team is a forward-thinking and innovative service that is community based and works in a fully integrated way, with physiotherapists, occupational therapists, and community matrons all working side by side to ensure an integrated and holistic approach to the care of people accessing our service.
The successful candidate will have the opportunity to work alongside and attend training with nursing and therapy colleagues within the team. Care closer to home is a multidisciplinary team offering valued, challenging and rewarding role within the team and will benefit from the support of a friendly hard working Integrated Community Team consisting of Nurses, Social Workers and therapists as well as other assistant practitioners and Nurse associates.
The working hours are 9-5, Monday to Friday ( this is a new service and this can change )
Being a car driver is desirable due to the busy fast paced workload of community nursing.
You will be expected to carry out clinical visits to patients in their own homes and residential homes under the direction of the registered nurse delivering a variety of nursing skills to the population, undertake continence assessments within both a clinic and home environment and work as part of the multidisciplinary team when needed too.
The role will require you to be computer literate as all patients records are now electronic (RIO) and previous experience of administration duties will be advantageous.
The successful applicant must be flexible, a good team worker, used to working under pressure, have excellent interpersonal and IT skills and be able to communicate effectively with patients, carers and other members of the MDT.
We are interested in hearing from both assistant practitioners and nurse associate’s with proven clinical and communication skills to work as part of our friendly, supportive and innovative team. Previous experience in a community setting in the NHS is highly desirable but not essential.
The successful applicant will demonstrate the ability to utilise a range of skills and experience gained in clinical practice to assist the registered nurse to undertake comprehensive and holistic assessments.
Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement - Outer London
This post also attracts payment for High Cost Area Supplement of 15% of basic salary, with a minimum of £4,551 and maximum of £5,735, pro rata for part time staff
