Leeds and York Partnership NHS Foundation Trust is the lead provider for perinatal services for the Yorkshire and Humber provider collaborative which goes live in April 2025. In autumn 2025 there will be a further bed expansion to the existing and only mother and baby unit in Yorkshire and Humber, which will allow for inpatient care to be provided for mothers and their infants closer to their home. The mother and baby unit provides acute inpatient care for women and their infant (from 32 weeks of pregnancy up to 12 months postnatal) who are suffering from a moderate to severe mental illness.
An exciting opportunity has arisen for an enthusiastic and motivated individual(s) to join our innovative, dynamic and forward-thinking perinatal mental health referrals management service. The Referrals Management Service was established to coordinate referrals from across the region for the Yorkshire and Humber Mother and Baby Unit. This is a new and developing part of the existing perinatal mental health community provision, providing opportunities for the successful candidate to contribute to the development of the service.
Main duties of the job
The post holder will be required to work across the perinatal community service and mother and baby unit, whilst liaising with acute mental health wards, community mental health services, health professionals and social care across the region. The post holder will attend CPA meetings for service users placed out of area and in acute mental health wards. Experience of triaging referrals and liaising with professionals would be advantageous for the role.
Some of our families have very complex social circumstances and this needs sensitive handling alongside the management of the mental illness and the ongoing safeguarding of mother and infant. The post holder will lead on partners assessments and the initiation of early help plans to support the wider family.

The successful candidate will hold a qualification as a Registered Mental Health Nurse, Occupational Therapist or Social Worker with experience of working in mental health care. They should have experience of working autonomously and as part of a multi-disciplinary team with an ability to think strategically and demonstrate excellent time management skills. This is an exciting opportunity to join a team that is committed to working in a multi-agency recovery-focused way with mums, families and wider social networks across all areas of our service.
