As a Clinical Pharmacist in Surrey Heath you will join the growing clinical pharmacy and multi-disciplinary PCN team. Working across a nominated general practice to support the responsibilities and deliverable of the Surrey Heath Primary Care Network. Initially, your time will be allocated on rotation across one nominated GP practices and also to the PCN Care Homes Team, to enable you to build your clinical skills and confidence and benefit from the diversity of portfolio working. You will be responsible for supporting the practice(s) to implement effective medicines management, identifying areas for improvement, and initiating and managing change. In addition the role will be responsible for supporting the Primary Care Network to implement the medicines optimisation requirements of the new Network Contract, Direct Enhanced Service for enhanced health in care homes and other PCN medicines management responsibilities. Main duties of the job This is a training role which requires the successful applicant to complete a CPPE course, therefore please ensure you understand the training requirements. These roles have been designed to be interesting and fulfilling roles, providing a comprehensive range of appropriate clinical pharmacy services to general practice in Surrey Heath. The post holders will undertake resident/patient-facing activities, and associated medicines management support activities, to ensure effective and efficient use of medicines within the practices and across their registered populations. The post holder will be supported to help develop the role and work to improve the care and outcomes for the patients across Surrey Heath PCN. Post-graduate clinical diploma level or equivalent training/experience.
