The post holder will encompass building a repour with GP surgerys, Livewell services and working with the voluntary agencies within the community. Supporting adults over the ages of 18, completing assessments, formulations and interventions, including group work. The post holder will keep up to date electronic records of all patient and staff activity, ensuring these are kept in a safe place, in accordance with NMC or HPC guidelines, and Livewell Southwest policies including record keeping, confidentiality, Data Protection Act and the Mental Health Act and professional conduct The post holder will be aware of NMC standards or own professional standards and Livewell Southwest guidelines on Mental Health Act and Data Protection Act. The post holder will, in conjunction with other staff from the service will provide assessments within clinical settings or home environment to determine mental health needs and provide consultation and advice to referrers and other agencies regarding alternative appropriate services when the patient does not need the support of the CMHT pathway. The post holder will provide evidence-based recovery focussed interventions for patients and carers on an individual or group basis in conjunction with our partner agencies. The post holder will advise, in conjunction with Drs and the Non-medical prescriber, on matters relating to medication. The post holder, in conjunction with the MDT and other relevant service areas, will develop and deliver psychoeducational packages and training related to the management of common mental health disorders to support third sector agencies to assist them to support people with common mental health problems who do not require ongoing support from the community mental health teams. The post holder will provide consultation to primary care and third sector agencies to ensure that those people who require mental health support are identified and are referred onto the appropriate services in a timely manner. The post holder will provide assessment, support and guidance for those people who present frequently or present with complex problems but who do not meet the criteria for secondary mental health care. The post holder will be involved educating primary care staff on the role of the Mental Health Assessment and Intervention Service. The post holder will develop and maintain links with primary care. The post holder will support the team manager with the daily management of the team, taking a senior lead in the absence of team manager. The post holder will take an active role in the development of the team, attending team away days, business meetings and attendance at MDT meetings. The post holder will work with MAIS to develop interventions which support the MAIS and wider mental health services to become NICE guideline compliant. The post holder will be required to attend any relevant working groups as directed by the manager in relation to the educational aspect of their role. The post holder will work under the direction of the MAIS manager to agree how the education packages will be delivered The post holder will support the development and delivery of training for patients, carers and partner agencies in agreement with the MAIS manager. The post-holder will take part in the clinical audit of the team. This will include annual Records audits. The post holder will be required to keep up to date this will include Mandatory training and developments in mental health care. The post holder will build relationships with third sector agencies, other primary care services and secondary mental health services. This role will involve regular time spent using computerised systems.
