An exciting opportunity has arisen for a full-time Band 6 care co-ordinator to join our team. We are looking for a versatile, motivated, committed, and experienced mental health nurse to work with adults over the age of 65 who are living with a functional and/or organic illness.
We are a supportive, experienced, and highly motivated multidisciplinary team. The successful candidate will be committed to working within the recovery model and dedicated to improving the well-being of service users and their carers. As a member of the team, you will manage the holistic needs of people on your caseload, undertake a range of assessments, and provide a variety of psychosocial interventions. As a care coordinator, you will also take responsibility for commissioning care using Local Authority systems and procedures.

Main duties of the job
To deliver evidence-based holistic care to clients within the context of a multi-disciplinary, multi-agency Community Mental Health Service.
To take responsibility for a clinical caseload providing comprehensive assessment, care, and treatment to clients referred to the Community Mental Health Team in accordance with Parts 2 and 3 of the Mental Health (Wales) Measure 2010.
Foster robust working relationships with all partners and key stakeholders, particularly Local Authority, Primary Care, and the Third sector.
Contribute to the continuing improvement and development of community mental health services.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

Qualifications
* Demonstrable post-registration development through relevant courses and study days
* Psychological therapies
Experience

Special Knowledge
* Demonstrable post-registration knowledge through experience as a qualified mental health nurse / attendance of formal / informal courses / training

* Knowledge of safeguarding procedures

Salary: £37,898 to £45,637 per annum, pro rata'd if part-time.
