Responsibilities include, but are not limited to:
* Conducting thorough assessments of patients’ health conditions and medical history.
* Developing, implementing, and reviewing individualised care plans.
* Prescribing medications and treatments within the scope of practice and in accordance with clinical guidelines.
* Monitoring the effectiveness of prescribed treatments and making necessary adjustments.
* Providing patients with comprehensive information about their medications, including potential side effects and interactions.
* Working collaboratively with other healthcare professionals, including doctors, nurses, and pharmacists, to ensure cohesive patient care.
* Maintaining accurate and up-to-date patient records and documentation.
* Participating in multidisciplinary team meetings and case conferences.
* Educating and supporting patients in managing their health conditions and medications.
* Staying informed about the latest developments in pharmacology, prescribing practices, and healthcare regulations.
Requirements:
* A relevant professional qualification (e.g., registered nurse, pharmacist) with additional training and certification as a Non-Medical Prescriber.
* Current registration with the appropriate regulatory body (e.g., NMC, GPhC).
* Proven experience in a clinical setting, with experience in prescribing medications.
* In-depth knowledge of pharmacology, prescribing practices, and relevant healthcare regulations.
* Excellent clinical assessment and diagnostic skills.
* Strong communication and interpersonal skills.
* Ability to work effectively as part of a multidisciplinary team.
* Enhanced DBS dated within the last 12 months.
* Willingness to complete an enhanced DBS if you do not currently hold one.
