An exciting opportunity has arisen for an enthusiastic, well-motivated and flexible Band 4 Therapy Assistant Practitioner to join our Urgent Community Response and Virtual Ward Services across South Tyneside.
This role is based with the Recovery at Home team at Clarendon.
The team consists of an extensive MDT which includes nurse practitioners, occupational therapists, physiotherapists, health care assistants, therapy assistant practitioners, and administrative staff. The team also works closely alongside partner agencies including those from the local and voluntary sector.

Main Duties of the Job
This role would involve supporting people in their own homes who are either acutely unwell or have recently been discharged from hospital and require enhanced intervention and short-term support for their physical and functional health needs to remain at home.
The service covers 24 hours, 7 days per week, it is fast-paced, and you would have the opportunity to see a wide range of patients who present with various conditions.

Job Responsibilities

* To work within the Recovery at Home Team to provide care to patients in their own homes or in Intermediate Care settings to prevent admission/readmission to hospital or long-term care and support the appropriate early discharge of patients over a 24-hour period.
* To support programmes of rehabilitation and health promotion to help maintain the patient in their own environment, thereby reducing the need for hospital admission/attendance.
* The post holder will also carry out planned health and/or social care interventions as designated by the lead clinician and will work unsupervised in patients' own homes, residential, nursing, and other intermediate care facilities, reporting back patient progress and informing the lead clinician of any problems.
* The post holder will work flexibly across boundaries of care to support patients in their own homes based on identified need.
Person Specification
Skills and Knowledge
* Excellent communication skills (verbal and written).
* Knowledge of the role of the physiotherapist/occupational therapist and rehabilitation.
* Basic IT skills.
* Ability to prioritise workload.
* Ability to work independently following guidelines from qualified staff.
* Ability to motivate people.
Other
* To meet the travel requirements of the post.

Experience
* Evidence of interaction with the public or patients in a care setting.
* Experience of working as part of a team.
* Experience of working as a physiotherapy assistant, health care assistant, support worker, or other relevant role such as fitness instructor.
* Experience of working using own initiative.
* Experience of group working and teaching.
* Recent proven experience of working with clients in a hospital or NHS organisation.
* Previous experience as a technical instructor.
* Experience of working with individuals who have communication difficulties such as hearing impairment or dementia.
* Competency training in handling patients with neurological or musculoskeletal impairment, carrying out individual exercise programs and group exercises.
Physical Skills
* Able to meet the moving and handling requirements of the post; frequent moderate effort required.
Qualifications
* GCSE in English and Numeracy skills to level 1 or equivalent.
* Practical and teaching knowledge to NVQ 3 in health-related subject or relevant past experience.
* Recognized academic qualification in a health-related topic.
* Record of personal development and reflective practice.
* Attendance at relevant professional courses to diploma level.
