Job Title: Adults Social Worker CMHT Location: Dorchester/Wareham Contract Type: Temporary Salary: £35 a hour Job Summary We are seeking a compassionate and dedicated Adult Social Worker to join our team. The successful candidate will be responsible for providing support and assistance to adults facing various challenges, including mental health issues, physical disabilities, and social difficulties. The role involves assessing clients' needs, developing care plans, and coordinating with other professionals to ensure the well-being and improved quality of life for our clients. Key Responsibilities: Assess Client Needs: Evaluate clients' physical, emotional, and social needs through interviews, assessments, and observations. Develop Care Plans: Create individualized care plans that address clients' specific needs and goals, and regularly review and adjust these plans as necessary. Provide Support and Counseling: Offer emotional support, counseling, and guidance to clients and their families to help them cope with challenges and improve their well-being. Coordinate Services: Work collaboratively with healthcare providers, community organizations, and other professionals to coordinate services and resources for clients. Advocate for Clients: Advocate on behalf of clients to ensure they receive the necessary services and support, including housing, healthcare, and financial assistance. Maintain Records: Keep accurate and up-to-date records of client interactions, assessments, and progress. Crisis Intervention: Provide immediate support and intervention in crisis situations to ensure clients' safety and well-being. Participate in Meetings: Attend and contribute to multidisciplinary team meetings, case conferences, and other relevant meetings. Qualifications: Education: Bachelor's or master's degree in social work. Licensure: SWE registration Experience: Previous experience working with adults in a social work or related setting is preferred. Skills: Strong communication, interpersonal, and organizational skills. Ability to work independently and as part of a team. Empathy, patience, and resilience in dealing with challenging situations.
