To work as part of a MDT liaising closely with medical, therapy and nursing staff across the network, to ensure pathways/protocols are followed and best practice delivered. Identify patients that meet HART criteria and gather relevant information from board round and MDT discussions. Attend handovers/MDT meetings as appropriate. To complete a robust OT assessment of patients. Monitor, evaluate and review OT interventions within a multidisciplinary framework and modify as necessary. Be responsible for identifying barriers to discharge/recovery at an early stage, and act on these within the MDT to ensure smooth patient journey. Work with external agencies and partners to ensure a seamless transfer of care. Provide advice to junior members of staff, therapy assistants and students on a range of issues pertinent to safe and effective interventions and discharge planning. Order and advise on a range of specialist disability equipment utilising correct eligibility criteria. Ensure communication of all plans/findings to patient and/or family/carers as appropriate and signpost to additional support services. To maintain and update registration with the Health and Care Professions Council (HCPC) including maintenance of a personal portfolio and to work within the scope of a professional practice as laid out by the HCPC code of professional conduct. Be a positive role model for all staff, patients and their carers. To work in accordance with Severn Major Trauma Network and North Bristol NHS Trust policies and procedures. Support North Bristol NHS Trust in the event of an internal or external major incident or mass casualty event, or pandemic. Clinical Governance To contribute to and work within the HART Quality and Governance framework including identification and resolution (where appropriate) of governance issues, support audits and pathway evaluation. Identify and lead service improvement programmes to achieve service development as agreed with clinical and managerial leads. Be aware of and work within the policies of North Bristol NHS Trust, carrying out such duties as may be required and are consistent with the responsibilities of the band. Management and Leadership To be a clinical link between Ward Sisters, Consultants and Rehabilitation department leads within North Bristol NHS Trust and the Hyper Acute Rehab Team. Exercise leadership and inspire colleagues to improve standards and quality to develop professional practice. Be confident in challenging concerns/queries regarding patient care. Training Support a variety of training programme delivered by the HART as directed by clinical and managerial leads. To contribute towards health promotion for all patients providing educational advice and support. Mentor/coach junior staff members as directed. Be willing to take advantage of training opportunities.
