Community Mental Health Nurse - Memory Treatment
An exciting opportunity has arisen for a motivated and compassionate Mental Health Nurse to join our friendly Memory Assessment and Treatment service (MATS).
As an integral part of this role, you will be supporting people through their journey from referral to post-diagnosis of dementia. You will provide screening, triage, and post-diagnostic support, including support for carers, signposting to other agencies, and monitoring of medication. You will be a part of a team which is looking at exciting ways to improve efficiency whilst maintaining the interests of the service users at the centre of our values.
Main Duties of the Job
As a memory assessment and treatment nurse, you will be working in a busy friendly team to provide assessment, triage, and monitoring of medication, and signposting to other services which can assist the service user to live well if a diagnosis is indicated. You will work within the framework of our operating procedure to ensure that we are meeting Key Performance Indicators and the needs of the local population for contributing to dementia diagnostic rates as set out by NHS England.
You will work autonomously to see people in clinics throughout West Suffolk and also in their home if they are unable to attend clinic. You will have good assessment and observational skills, a kind and compassionate approach, and a true interest in supporting older people.
You will liaise with other providers (e.g., GP, social services, charitable services, carers' matters) to ensure people receive the most appropriate support immediately and in the longer term.

Person Specification
Qualifications
* Nursing Degree or equivalent
* Valid registration with NMC
* Mentorship qualification or willingness to undertake mentorship training
Experience
* CPA implementation
* Experience in appropriate area
Skills & Other
* "Prevention and Management of Suicide" training
* Working knowledge of Care Programme Approach
* Ability to travel independently
