We are looking for skilled and outstanding clinicians to join our UCR service within the Community and Growing the Friarage Collaborative. The community sister in UCR will have physical assessment and disease management skills and will evidence management of acute/chronic clinical conditions, multiple co-morbidities and poly-pharmacy, within the home environment, ensuring an optimum standard of patient centred care. They ideally will be an independent non-medical prescriber, applications will be considered if you do not possess this qualification. Experience of working in a community setting, working within an emergency assessment unit or rehabilitation would be an advantage and we encourage informal visits to the areas. Main duties of the job The Urgent Community Response Sister/Charge Nurse will work as part of the multi disciplinary team to continuously assess the needs of the individuals with long term conditions, complex health needs and acute episodes of ill health requiring an urgent 2 hour response, and agree care plans,utilising the single assessment process. The post holder will work autonomously as part of the Community UCR and hospital at Home to provide a comprehensive assessment and patient centred care package, utilising support services and liaising with other agencies to ensure the needs of this client group are met.
