An exciting opportunity has arisen for a registered healthcare professional to work as a Quality Matron for Palliative and End of Life Care as part of the nursing and quality team and palliative care management team, leading on the quality agenda to include safety, patient experience and clinical effectiveness.
The post holder will provide clinical leadership for palliative and EOL care across inpatients, specialist nursing and the community.
The post holder will influence and be the lead on the development of the patient pathway for EOL and Palliative Care. This will be the combined pathway encompassing the Specialist Palliative Care unit, Specialist Palliative Care nurses and the Community Nursing and Therapy teams.
Ideally you will have existing leadership experience at a senior level and display excellent communication, problem solving and leadership skills and the ability to manage effectively challenging and changing situations.

About Us
The Community Specialist Palliative Care Nursing team works across North, South, Central and West Norfolk to provide specialist palliative care support, including complex symptom management and psychological care. There is also an out of hours rapid response service and a 24/7 advice line for patients, carers and healthcare professionals.
The team works closely with the Specialist Palliative Care medical team and the Priscilla Bacon hospice, as well as working closely with primary care, community nursing, inpatient and rehabilitation teams to provide palliative specialist support to the most complex palliative and end of life patients.

Job Responsibilities

5. To provide leadership for delivering quality improvement in relation to safety, patient experience and clinical effectiveness, embedding the ethos of a safety culture.
6. To provide oversight to the Community Hospital Inpatient Units and the PLACEs across the community and inpatient teams to include leading innovation and learning from incidents.
7. To lead on the quality and safety experience for patients across the Trusts internal service pathways for the specialist and community palliative services.
8. To ensure a responsive and proactive approach to managing and learning from concerns and complaints.
9. Contribute to the management of the clinical audit process and review of relevant internal incidents.
10. Working with the Clinical Quality Director to ensure a robust risk management system with appropriate escalation.
11. To lead on the quality and safety experience for patients, for those internal service palliative pathways that interface with pathways external to the Trust.
12. Lead the continuous improvement required for harm free care, safeguarding and infection control, monitoring practice and compliance.
13. Create and promote a learning culture supporting a change to practice proactively when the need is identified.
14. To work with other partners including the Integrated Care Service (ICS) to deliver high quality care.
15. To explore current practice through quality improvement, identifying areas for development and working with staff in planning the appropriate change using research and experience to evaluate the outcomes.
16. To coordinate practice, procedures and systems for clinical areas focusing on palliative and EOL care.
17. To provide professional leadership providing a strong clinical presence and developing clinicians and clinical practice, assuring the provision of a safe, effective, and evidence-based service to patients.
18. To be visible, accessible, and approachable to staff, patients and the public to ensure that open and honest communication channels are created and sustained.
19. To provide oversight of the Trusts policy compliance in relation to Palliative and EOL care and monitor and horizon scan for changes to National policy.
20. To lead on the implementation of assurance and action plans from the Quality Assurance Tool (QAT).
21. Ensure that the learning from National incidents/events relevant to Palliative Care (SARs, enquiries) is embedded in the Trust and shared with the relevant teams.
22. To lead and ensure high standards of practice in line with standards of care and other Government targets are embedded in practice.
23. To work with the specialist palliative care inpatient team, community specialist palliative care teams, intermediate care units and PLACE community nursing and therapy teams in assessing the clinical skills required delivering a safe and high quality service.
24. To provide advice as required, both internally and externally.
25. To act as a coach and mentor for members of staff both formally and informally.
26. To maintain a clinical profile through embedding processes for assuring quality.
27. To lead on infection control initiatives within own sphere of influence, ensuring that all staff within the organisation comply with Trust and NHS policies.
28. To support the implementation of audit, NICE guidance, and relevant standards.
29. Ensure that a multi-disciplinary approach is taken to raise standards in relation to Palliative and EOLC across the Trust.
30. To ensure key practices are embedded to support high quality care for fragile patients, including dementia and end of life.
31. Ensure NEWS2 and RESPECT are adopted, and staff are supported to deliver to a high standard.
32. To ensure staff that are line managed by the Quality Matron have individual performance reviews and personal development plans.
33. To assist in the resolution of any conflicts within the clinical area, resolving informal complaints locally and assisting in the investigation of and response to formal complaints.
34. To investigate accidents and incidents and respond appropriately to support the Patient Safety Incident Response Framework (PSIRF) principles.
35. To ensure there is an effective and efficient use of resources within the clinical areas and will be an authorised signatory.
36. To work with service leads to ensure that patients follow agreed pathways of care and are supported towards safe and effective discharge.
37. To be responsible for ensuring the Health and Safety measures are implemented and adhered to.
38. To work with relevant service leads to monitor the clinical staffing establishments ensuring timely recruitment and appropriate selection of staff.
39. To participate in the audit process to improve patient care delivery.
40. To provide appropriate guidance and support to staff in line with the Trusts disciplinary and sickness procedures.
41. To support, identify and supervise the implementation of environmental standards within relevant clinical areas.
