East Bedford Primary Care Network have an exciting opportunity for a hardworking care co-ordinator to join our primary care network on a full-time basis, however part-time can be considered if over 24 hours per week. The role covers the following GP practices in Bedford; Putnoe Medical Centre Linden Road Surgery London Road Health Centre Cauldwell Medical Centre The post-holder will help us to improve the care provided to some of our most vulnerable patients. The main duties of the role include: Working with GPs and other professionals within the four practices which form the PCN to identify and manage a caseload of patients. Conducting health reviews of patients with dementia, mental illness or learning disability. Using knowledge of locally available support and services to develop care plans with patient, focusing on prevention. Supporting the coordination of all key activity, including access to services, advice and information. Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date. For further information, please see the job description attached. About the Candidate If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you. The ideal applicant will have the following skills, experience and qualifications: Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups. Experience of working with patients with dementia, mental illness or a learning disability. Familiar with local resources and services, including how to access them. Motivated by helping people, with care and empathy. Able to deliver person-centred support in a non-judgmental way. Experience of completing holistic person-centred care planning assessments. Strong listening and communication skills. Diploma Level 2 in Health and Social Care, or an equivalent qualification (desirable not essential).
