Registered Nurse - Hospice at Home
Salary: £31,807-£37,960 per annum WTE based on 37.5 hours week, plus enhancements.
Full Time and Part Time work available.
Phyllis Tuckwell is recruiting Registered Nurses to join our expanding Hospice at Home team. We provide a responsive service for patients in their homes and work closely with community nursing teams to offer dignified, compassionate, and co-ordinated care for patients and their families.
Main duties of the job
We are looking for proactive Registered Nurses who are committed to learning and developing skills in palliative and end of life care, and medicine management. The successful candidates will work in an integrated way within a team and with partners. The successful applicant will be passionate about working effectively for the benefit of patients, carers, and families.

Post: Hospice at Home Registered Nurse
Accountable to: Team Lead, or designated other lead
Job Purpose:
The postholder will maintain the highest standards of nursing care with patients, their families and carers in the community setting. This role will involve providing holistic assessment, identification of patient needs, care planning and care provision jointly with other multidisciplinary team members. The service is complementary to, and dovetails with, other health care services within Guildford & Waverley, North-East Hampshire & Farnham and Surrey Heath, so that people who use our service receive co-ordinated and person-centred care.
The role requires a commitment to enabling choice and providing respectful and dignified support for people as they approach the end of life and to their families and carers. The postholder will contribute to a safe, effective, and efficient culture by participating in risk assessment and by working in accordance with Phyllis Tuckwell's (PT) policies and procedures.
Opportunities exist to rotate from Hospice at Home onto the In-Patient Unit to further develop your knowledge and skills in palliative and end of life care, and enhance awareness of the patient and family experience of end of life care.
Responsibilities
Clinical
* To assess, plan, implement and evaluate palliative care interventions based on an agreed plan of care, that supports quality and timely care delivered to the patient and family or carer, in line with their needs.
* To be proactive and flexible in response to the delivery of care within the home setting, assessing change, risks, and gain an understanding of preferences and wishes.
* To work closely with PT colleagues and the District Nursing team in the provision of personalised holistic care to patients, their families, and carers.
* To ensure quality standards are maintained and constantly re-evaluated, in order to provide the highest standards of holistic care.
* To ensure patient records are correctly maintained in accordance with NMC guidelines and PT policy.
* To be able to demonstrate an understanding of issues relating to the ethical aspects of palliative care and their application in the workplace.
* To report risks, accidents, incidents, negative feedback, and complaints to the Team Lead/Manager same day.
* To understand and adhere to Phyllis Tuckwell's and the NHS Community Nursing's Medicines Management policy, in relation to the correct and safe handling, usage, and administration of all drugs.
Professional
* To understand and work within relevant legislation and PT philosophy, mission and values.
* To be mindful of own professional behaviours, conduct and practice, when working as part of a multidisciplinary team representing PT providing holistic care, information and support to patients, their families, and carers.
* To liaise and develop effective relationships with external health care professionals and other agencies ensuring patient continuity across all care settings.

* To ensure clinical practice is current and in line with best practice.
Managerial
* To ensure a safe working environment is maintained, in accordance with PT Governance, Risk Management, Fire, Health and Safety policies, reporting any potential hazards to the line manager.
* To report any feedback, concerns or complaints same day from patients, their relatives, or other advocates, to the Team lead/Line Manager as appropriate.
* To ensure that the nursing resources are used wisely.
* To ensure team cohesion and spirit is maintained, through ongoing supportive relationships and consideration of colleagues at all times.
* To adhere to the lone working policy in the community.
Training and Development
* To develop an area of clinical expertise and disseminate to team members as a resource.
* To support the professional development and safe practice of others.
* To maintain up-to-date clinical experience and knowledge and be able to provide evidence of this for nursing revalidation.
* To act as an assessor and mentor to students who need to develop skills and acquire experience in the field of palliative care.
* Through PT annual appraisal, encourage professional growth and identify areas for personal and professional development.
* To strengthen and develop clinical skills across all care settings in conjunction with line manager.
Research and Audit
* To participate in research and audit within the PT environment as appropriate to the service area.
* To implement findings of evidence-based practice.
