Job summary
Lowestoft Primary Care Network are seeking a health and wellbeing coach, for a 12 month fixed term contract, on a full time basis. Although the successful candidate may be required to work in any Lowestoft Practice. This position will mainly be based in Alexandra Road and Crestview Surgery.
As a health and wellbeing coach you will work as a key part of the PCN multi-disciplinary team. You will consider the whole person in addressing existing issues and encouraging the proactive prevention of new illnesses, promoting lifestyle medicine approaches.
Health coaching revolves around using your coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.
The role is patient facing, varied, rewarding and sometimes challenging and may include supporting self-management education, peer support, case management and facilitating group consultations. You will take an approach that is non-judgemental, based on strong communication and negotiation skills. You will support personal choice and positive risk taking, while ensuring that patients understand the accountability of their own actions and decisions. Your role and skills will support and encourage the prevention of developing further illness, or the deterioration of existing long term conditions.
Main duties of the job
1. Coaching and motivate through multiple sessions using personalised goal setting to implement a patients personalised health and care plan for long term lifestyle and behaviour change.
2. Identifying frequent attenders and work with patients with lower activation scores to understand and increase their level of knowledge, skills and confidence to self-management their health conditions.
3. Referring to Social Prescribing Link Workers and the broader PCN MDT to enable patients to take control of their health and wellbeing.
4. Supporting return to work and participation in community activities/exercise to improve mental and physical wellbeing.
5. Addressing loneliness.
6. Supporting recovery from drug and alcohol addiction.
7. Running lifestyle group education sessions/webinars and virtual/group consultations.
8. Identifying when patient needs are beyond the scope of the role and referring to the appropriate health professional or organisation.
9. Proactively working with referral partners to maximise appropriate referrals and to promote the service.

Qualifications
Essential
10. Educated to GCSE level or equivalent.
Desirable
11. NVQ 3 or equivalent and/or relevant basic/first level professional qualification.
Experience

12. Be willing to attend a 4 day training course delivered by an accredited Personalised Care Institute training provider.
13. Experience of working within a social care and/or health setting, working with multi-disciplinary teams.
14. Demonstrable experience of effective planning and organisational skills to deliver targets to deadlines.

15. Experience of multi-agency working and signposting to appropriate support.
16. Experience of working with people with diverse health and social care needs.
17. Experience of using recognised assessment tools.
