The post will help to reduce delayed discharges in the acute hospital and also waiting times in the community and enable patients' families/carers and supporting services to provide appropriate interventions in a timely manner. The Consultant will work closely with the Frailty team and the Admiral nurses who provide post-diagnostic support when the patient is an in-patient and then refers to the appropriate out-patient post-diagnostic support.
