Working closely with the Diabetes Specialist Nurses to perform comprehensive assessment of nursing needs, plan, implement and evaluate care delivery according to changing health care needs. Providing individualised education to people with diabetes and carers to support diabetes management and promoting healthy lifestyle. Supporting people with diabetes whilst they are in hospital within Aintree hospital, improving the patient safety, experience and outcomes and timely discharge back to place of residence Collect, collate, evaluate and report information, maintaining accurate person centred records. Involve people with diabetes and carers/relatives in the planning and delivery of care. Clinical sessions in a variety of settings including ward based reviews, outpatient clinics, and community clinics. Work collaboratively with other professionals and agencies to ensure individual person needs are met, especially in relation to on going care needs and discharge arrangements. Establish and maintain effective communication with people with diabetes and carers/relatives. Managerial/Leadership Provide specialist, expert clinical knowledge to clinical colleagues, people with diabetes and carers / relatives. Undertake clinical supervision of nursing colleagues on an individual or group basis Participate in the development of services and assists the Nurse Consultant to implement change. Ensure the effective and efficient use of physical and financial resources. Monitor health, safety and security of self and others and promote best practice in the ward area. Education and Development Provide education and development opportunities for others including individual education and education of other health care professionals. Develop own expert clinical knowledge and participate in the development of others clinical knowledge. Ensure own compliance and compliance of others with regard to mandatory training and revalidation requirements.
