You will be required to work as part of our developing Neighbourhood team to lead and co-ordinate nursing teams to provide complex nursing care to adults in their own home and in a variety of settings within the community. You will work closely with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patient care is effectively co-ordinated, demonstrating a positive impact on our patients.
