We are looking for an experienced Social Work Practitioner who will be our Social Work Senior Practitioner in the hospital discharge team.
As a senior practitioner you will work directly to the Team Manager for Mental Health and the Hospital Discharge Team within Blaenau Gwent.
Access to a vehicle for work purposes is a requirement and the successful post holder will be designated an agile worker which will mean that they will work both at our Social Care buildings and from home.

Blaenau Gwent County Borough Council are excited to have developed a lead practitioner post to support our most vulnerable residents. We are looking to recruit an ambitious and proactive individual who will be an ambassador for our vulnerable adults ensuring that they are appropriately safeguarded and that as statutory partners we support their views, choices and act in their best interests always. The successful candidate will be an experienced social work practitioner who has a passion for working within this field, who has a sound knowledge of relevant legislation and is confident in using this knowledge to develop and embed across our wider social work teams.
As a senior practitioner you will also be expected to:
• Supervise relevant staff including where appropriate the allocation of work and other managerial duties
• You will represent the Adults Services Department at relevant training events, internal and external meetings and conferences
• Lead, develop and deliver relevant training in relation to legislative changes
• Manage budgets
• Comply and adhere to the Authority’s policy statement on Health, Safety and Welfare at Work and the Corporate Equality Policy
All applicants will be required to:
• Hold a recognised social work qualification and preferably a Best Interest Assessor award
• Be registered with Social Care Wales
• Have significant post-qualifying experience of statutory adult care.
• Budget management experience
• Have experience of direct involvement with Adult Safeguarding, Mental Capacity, Complex Health Care legislation, it is desirable to have experience of Mental Health.
• Be experienced in chairing complex meetings with relevant statutory partners
