Our HomeFirst Health Care Assistants assist registered nurses and therapists to implement assessed care needs and helping maintain the patients rehabilitation and reablement progress. HomeFirst Health Care Assistants provide and assist with the care of people within their home environment, working with individuals and their families to ensure a person-centred approach is carried out, which includes the participation in clinical care procedures e.g. recording observations and application of topical cream, as well as helping patients / individuals with personal care tasks including assisting with meals and drink preparation if required. Their role and the actions they take support admission prevention by responding to incidents of carer breakdown, provide rapid support for individuals to prevent admission to hospital.
