As part of Mersey Cares Operational Transformation Programme an exciting opportunity has arisen for an enthusiastic and motivated clinical pharmacist to provide Medicines Management support to community specialist nursing teams.
The post holder will work as an advanced clinical pharmacist supporting with queries and medication reviews for Specialist Nursing Teams (such as the Frailty, Heart Failure, Community Palliative Care Teams) and the Urgent Community Response Team.
The post holder will complete medication reviews for patients to support them to get the best possible outcomes from their medicines. This will involve analysing and interpreting highly complex clinical situations and communicate effective medicines optimisation solutions. They will support an integrated approach to medicines optimisation, helping to ensure patients have access to the Medicines Optimisation support they require to improve patient outcomes. The post holder will work and communicate effectively within a multi-disciplinary team.
The postholder will provide specialist information, education, training and advice to members of the multi-disciplinary team to maximise pharmaceutical care.
You will receive clinical supervision and peer support from a large Medicines Management team within Merseycare NHS Foundation Trust.
1. Support with queries and Domiciliary Medication Reviews for patients under review by the Urgent Community Response Team. This will also include supporting existing Medicines Management pharmacists and technicians already embedded within these teams.
2. Developing and delivering a clinical pharmacy service within community physical health services to specialist nursing teams (such as the Frailty, Heart Failure and Community Palliative Care Team).

1. To conduct medication reviews in patients on high-risk medicines, complex medication issues and/or those with medication adherence issues or another medicines related issue. This may involve travelling to the patients’ normal place of residence to carry out a domiciliary assessment.
2. To involve patients and their carers in decisions about their medicines to jointly develop a medicines care plan to help patients to get the best from their medicines.
3. To manage complex medicines optimisation problems within the team and across the interface.
4. To identify and communicate recommendations from the medicines assessment to the patient’s own GP & multi-disciplinary team (MDT) as appropriate. Where appropriate discuss with the secondary care prescriber and pharmacy team, and community pharmacy.
5. To ensure agreed actions related to medicines are implemented and have appropriate follow up plans.
6. To participate in multi-disciplinary Team (MDT) meetings, providing expert pharmaceutical advice on complex medicines related issues.
7. To work with inpatient health professionals, secondary care, community health, primary care and social care services to optimise patient centred care in respect to medicines.
8. To work with hospital & community pharmacy team(s) to identify and develop solutions to medicines related issues and improve communication about medicines at the interface.
