A fantastic opportunity has arisen for a Wellbeing Mentor to join the Smokefree Norfolk team. This is an exciting role working on a programme focused on reducing smoking rates in people who have a Severe Mental Illness (SMI). This initiative aims to improve the health and life-expectancy of this community and reduce health inequalities while furthering the NHS's Long-Term Plan. We are looking for highly motivated and imaginative individuals with excellent communication skills who can engage creatively and effectively with this group, working with clinical and administrative colleagues across departments and external agencies.
Smokefree Norfolk and Together for Mental Well-being are working in partnership with Norfolk and Waveney ICB to provide a smoking cessation service for Adults with Serious Mental Illness across Norfolk. The aim of the partnership is to offer additional support to people with SMI who want to quit tobacco by offering a social prescribing model of support and onward referral. Key elements of this role will be to deliver a clinical tobacco cessation service (full training given) to SMI patients and to support them on their journey to quit by reducing their tobacco consumption.
This is a fixed term position ending 31st March 2025.
Key Responsibilities
* Support individuals with serious mental illness (SMI) and complex needs to engage in the service and achieve their goals.
* Provide support using a fully flexible and personalised approach, acting as a support navigator to utilise community resources.
* Empower individuals to exercise choice and control, offering a person-centred service.
* Undertake sessions with individuals using person-centred planning tools.
* Act as a champion in areas such as psychological education and develop links with local services.
* Support individuals in areas related to mental and physical health as part of a multi-disciplinary team.
* Provide 1-1 advice and support to people with SMI who want help to stop smoking.
* Develop individual care packages to support smokers attempting to quit.
* Engage and liaise with Level 3 advisors, GPs, and pharmacies regarding clients’ needs.
* Work with IT systems to ensure accurate and timely recording of service user data.
* Participate in the staff appraisal and supervision scheme and continuing professional development activities.
* Communicate effectively with individuals from a wide variety of backgrounds.
* Contribute to the development of smoking cessation best practices for those with SMI.
