Keyaccountabilities
Quality To maintain a highlevel of quality in service provision by:
* Meeting quality expectations and agreedperformance criteria
* Participating andutilising management information and information collection systemsas appropriate
* Contributing to the continuousimprovement of the service.
Own Development Tocontinuously review own performance and development needs to assistgrowth and development by:
* Participating in open twoway dialogue duringPerformance Management meetings agreeing own task and developmentobjectives and reviewing these and overall performance against thecompetency framework and skills profile for the Support Workerrole.
* Participating in training and otherdevelopment opportunities as
agreed with your linemanager.
Health andSafety & Risk Management To ensure a safeworking environment for self and colleagues by:
* Ensuring good standard of housekeeping ismaintained in own area
* Ensuring riskassessments are completed whenappropriate
* Taking personal responsibility forown safety e.g. reporting concerns ensuring appropriatevaccinations and eye tests etc. areobtained
* Complying with all H&Spolicies and procedures including serious
untowardincidents and accident reporting
Compliance To ensurecompliance with internal and external standards and

Miscellaneous To undertakeany other duties reasonably requested by the
linemanager.
ServiceUsers To proactively deliver a high quality/personcentred service
provision that meets the needs of serviceusers by:
* Supporting individuals withtheir personal care which may include skin care bathing orshowering shaving oral care intimate care and continence (includingmanagement of bladder and bowels) dressing/undressing and haircare.
* Supporting individuals with housekeepingduties such as cooking cleaning laundry ironing washing dishes andrunning errands; carrying out these tasks in full when individualsare unable to do so.
* Promoting peoples rightsand responsibilities
* Working as an effectivemember of the team
* Providing advice andinformation for service users and others whereappropriate
* Delivering agreed support packagesto quality standards
* Liaising with externalprofessional groups e.g. GPs Social Servicesetc.
* Ensuring record keeping is maintained tothe required standard at all times and contributing to servicemonitoring requirements.
* As appropriatesupporting services users to take their prescribed medication ontime assisting them where necessary
* Ensuringfull risk assessments and risk management is deliveredeffectively.
* Ensuring that work is undertakenin line with Health and Safetyrequirements
* Assisting the facilitation ofeffective care pathways
* Supporting individualsto achieve goals and objectives as set out in their care/supportplans
* Supporting and enabling individuals toaccess community based services andfacilities
ServiceSupport To assist in the implementation developmentand delivery of the service by:
* Participating in the induction of newcolleagues when required.
* Delivering all taskson time and to agreed qualitystandards
* Participating in the service userreviews with line manager or othercolleagues.
ServiceDevelopment & Quality To workcollaboratively to develop the
service by:
* Ensuring the service and the wider organisationis represented in a professional manner at alltimes.
* Proactively contributing to continuouslyimproving the service by making positive suggestions providingconstructive feedback and assisting in the implementation of agreednew ways of working.
* Ensuring day to daydelivery of service provision embeds andextends
* The organisations person centredapproach.
Empathy andSupport To provide empathy and support to serviceusers by:
* Working with service usersin a personcentred way to meet needs
* personalgoals and aspirations
* Encouraging service usersto achieve their potential

Sector QualityStandards To ensure all services are delivered inaccordance
with recognised standardsby:
* Ensuring all servicesare delivered within CQC and/ or Supporting People standards asappropriate.
Movingand Handling Enabling individuals mobility includingmoving and handling transfers pushing wheelchairs assistingindividuals to use walking aids working with professionals such asphysiotherapists and occupational therapists checking equipmente.g. wheelchair seat cushions supporting service users on publictransport driving shared transport vehiclesand
individuals own vehicles.
Community Access Supportingindividuals to access the local and wider community visiting localamenities carrying out activities both on foot and publictransport.
