Please apply to join our team today Main duties of the job This is an opportunity for the applicant to demonstrate their ability to provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home. Our urgent (Rapid), community response team assess and provide individualised patient care, prioritising need to ensure our patients get the most efficient and clinically effective treatment in their own homes. This post holder will be skilled in promoting self care to our patients and have a drive for excellence as they join our community nursing services. Evidence based high quality care is our aim in Rapid Response as we actively support the wider community nursing and therapy teams visiting patients who require a new or urgent episode of care. This post holder will work in close collaboration with the wider Integrated Community Teams/GP surgeries and local acute trusts.
