Job overview
The Camden Community COPD and Home Oxygen Service wants to hear from you. CNWL NHS Foundation Trust is looking to recruit to a Band 3 Allied Health Professional Support Worker (Rehabilitation Assistant). We provide diagnosis, management, and rehabilitation for clients with respiratory disease, mainly Chronic Obstructive Pulmonary Disease (COPD) and those whom receive home oxygen therapy.
This is a fixed term contract for a full-time post for 8 months (or 4 days per week for up to 10 months). The majority of the post-holder’s time would be spent assisting in the organisation and delivery of Pulmonary Rehabilitation (PR) classes to patients. This is an exciting time for the team as we are working towards accreditation on the Pulmonary Rehabilitation Services Accreditation Scheme (PRSAS) in addition to being involved in a Health Inequalities project to increase access to PR.
Main duties of the job
* Leading some aspects of the PR class e.g. warm ups and cool downs, providing some of the education sessions, and helping with assessments, all with the support of a physiotherapist.
* Helping with the day-to-day running of the team alongside one other rehabilitation assistant and the administration staff, which may include some non-patient facing roles e.g. calibration of some of the medical equipment.
We are a friendly and supportive community-based team comprised of nurses, physiotherapists and rehabilitation assistants, all with experience in the respiratory area, and able to provide training and support for the successful candidate to work with patients who have a respiratory condition.
The post holder will be based at St Pancras Hospital, and may be required to work in a variety of settings, such as client’s homes, GP surgeries, clinics and hostels. We are looking for someone with excellent communication skills, who has had previous experience of working with clients with a health condition or in an exercise capacity, who is well organised, able to work independently and as part of a team and able to adapt to different clinical and non-clinical situations. Good IT skills are essential to this role, as some of the work is relating to booking clinics, doing online referrals, writing health care records and inputting data.

Detailed job description and main responsibilities
* To undertake health promotion for patients e.g. managing physical and mental health co-morbidities, alcohol and substance and smoking cessation.
* To work in conjunction and support qualified practitioners to deliver PR to patients with chronic lung disease, including: setting up/clearing away equipment; delivering aspects of the class e.g. warm up/cool down; conducting aspects of patient assessment e.g. field walking tests and patient observations; supporting the delivery of patient education.
* To work in conjunction and support qualified practitioners to produce client goals appropriate to them and involving them in the process at every opportunity and to undertake re-assessment and review of progress to implement change in management as required with support from qualified practitioners.
* To support the wider team by answering the telephone to triage patient queries and book appointments.
* Contributing to the effectiveness of the clinical services by providing general support to all team members including students. This may include some general administrative tasks and maintaining equipment essential for clinical care and contacting patients and arranging appointments.
* To assist induction of new or less experienced staff and students by demonstrating their own role and levels of accountability.
* To contribute to research projects/audits/surveys within the clinical specialty under the supervision of registered health care professionals.
* Undertaking all delegated work ensuring deadlines are met.
* To be able to engage in positive relationships with patients, carers and families and other staff based on respect and within appropriate boundaries.

Qualifications
* NVQ L3 (or equivalent)
* Numeracy and Literacy skills required to keep accurate records e.g. GCSE Maths and English (or equivalent)
* Care certificate
* Qualification in prescription of exercise
Previous Experience
* Previous experience working in at least one of the following settings: 1) Exercise 2) Rehabilitation 3) Community 4) Patient Care
* Previous experience in: Pulmonary Rehabilitation
* Previous experience in: Working with patients with long term conditions especially lung conditions
* Previous experience in: Supporting and delivering exercise classes and/or individual exercise programmes
Skills and Knowledge
* Able to communicate with the multi-disciplinary team, update others in the team and use handover effectively. Speaking face to face but also by telephone.
* Able to maintain accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the CNWL policies.
* You are expected to adhere to the Code of Conduct for Health Care and Support Workers and Adult Social Care Workers in England.
* Contribute to assessments and classes under the supervision of the Physiotherapist/Nurses.
* Able to work independently but follow the directions of care plans and registered staff.
* Experience delivering education talks to a patient or client group.
