Job summary
This is an exciting post to further embed the current offer that delivers digital support to patients combining personalised care and health inequalities priorities and principles. The successful candidate will help implement interventions that address inequalities in access, care and outcomes, creating sustainable change for patients. The aim of the pilot is to contribute to the digitalisation of healthcare by supporting those patients more likely to experience barriers to their care or to the benefits that digitalisation brings, as well as advancing the NHS Digital Inclusion strategy.
Main duties of the job
The post-holder will effectively lead and line-manage a small team working across all areas of the Trust, including wards, Hospital at Home and the Emergency Department and will work closely to, and in collaboration with, clinical colleagues.
As a Team Leader you will understand cycles of continuous quality improvement and service development and have a proven track record of service implementation.
The role combines patient-facing responsibilities as well as operational and strategic ones. The successful candidate will have a caseload and be expected to cover some of the services the Digital Inclusion pilot offers.
The role requires someone with exceptional organisational skills who can also demonstrate great communication, analytical, interpersonal and networking skills. A caring and sensitive attitude is essential as you will be communicating with patients, their families and carers.
The Digital Inclusion service falls within the Trust's Health Inequalities Programme. You will champion Personalised Care principles and help achieve better health outcomes for our patients and reduce health inequalities.
