Job summary
Thame Adult Community Health Care Team (ACHT) has an exciting job opportunity for a Health Care Assistant post. As a member of the Adult Community Healthcare Team, the post holder will be providing care alongside nurses and nursing associates within the district nursing. The overall aims of the Adult Community Healthcare Team are to avoid inappropriate admission into the acute service, facilitate timely discharge into the community, and enable individuals to be cared for in their home environment.
The post holder will be appropriately trained and supervised in various skills to work independently supporting the District nursing service.
Main duties of the job
The post will involve working in the community, supporting the community nurses in providing nursing care interventions, and promoting independence in the patient's home. Candidates should be experienced carers and be able to demonstrate experience or knowledge of supporting patients in their own homes. You will have excellent communication skills and the ability to work with a range of health professionals and informal carers. The ideal candidate will be flexible and adaptable, able to respond to healthcare needs of the local population. The service covers a 7 days a week rota which includes weekends and bank holidays. Ideally, candidates must hold an NVQ level 3 or equivalent in Health and Social Care.

EDUCATION, QUALIFICATIONS & TRAINING
Essential
* English and Maths Qualification. Either; GCSE A-C, Functional Skills Level 2, or GCSE 4 +
* NVQ Level 3 in Health and Social Care OR Diploma in Health and Support Level 3 OR Care Certificate in healthcare (Level 3)
SKILLS, ABILITIES & KNOWLEDGE

Desirable
* Physically able to perform tasks whilst kneeling, bending and able to handle patients and equipment safely

* Previous caring experience in a health or social care environment

* Previous NHS/Social care/Voluntary organisation experience
* Understanding of Integrated Team
