An exciting opportunity has arisen and Essex Partnership University NHS Trust (EPUT) are seeking to recruit to a Paediatric Liaison Health Visitor with an experienced, motivated and enthusiastic Health Visitor to join our friendly, supportive team, based in Rayleigh Clinic.
The Paediatric Liaison Health Visitor role will develop, promote and facilitate appropriate referral and liaison between Acute A&E hospital services, walk in centres, Urgent Care Centre, Health Visitors, School Nurses and other community practitioners as required. Acting as a professional resource and link between Acute, Community and Primary Care services establishing good communications within these areas including wards, out-patients, A&E, SCBU and maternity services.
The Paediatric Liaison Health Visitor will improve and facilitate robust methods of communication enabling professionals to become more effective in identifying vulnerability indicators in children, young people and families and ensure School Nurses/Health Visitors are involved in effective discharge planning arrangements of children as appropriate. The Paediatric Liaison Health Visitor will need to be able to work independently as well as within a team and be able to make decisions regarding safeguarding children.
To ensure facilitation of safe discharge needs of the complex patient work load inclusive of the Neo-Natal Unit and Paediatric A&E/PAU as required.
Develop, implement and evaluate service discharge policies and protocols to ensure appropriate services receive information of children who have had an in- patient episode or attended Emergency Departments irrespective of the reason for admission or attendance.
Participate in assessment, planning and provision of patient’s needs before discharge. Ensure teaching and development of parents/carers and nursing staff.
To ensure EPUT Community Services adheres to the Pan Essex Guidance for Managing Emergency / Unplanned Care Liaison Information.
To be the named single point of access to children’s community services for Southend University Hospital Foundation Trust (SUHFT) for children.
To represent EPUT at agreed multi-agency / disciplinary forums, discharge planning meetings/ward rounds as required to meet the remit of this position.
Ensure the timely transfer of information sharing from SUHFT hospital to community services as per the Essex Guidance.
For children living within South East Essex, information from A&E, Neptune Ward and NICU will be scanned onto SystmOne and a task sent to the relevant practitioner within 2 working days, Monday to Friday excluding Bank Holidays.

Provide Education and training to support staff within the unit to ensure children and young people have their emotional health and wellbeing needs met and that risk taking behaviours are identified and appropriately safeguarded.
The post holder must be able to understand complex health visitor Liaison issues and be able to assess risk for both patients, carers, families and staff.
Provide support to staff involved in complex cases this may include attendance at Discharge planning meetings off site, strategy meetings, professional meetings and other interagency/professional meetings as agreed.
Analyse and assess safeguarding situations appropriately and act accordingly.
To identify frequent attenders to the UCC / A & E and improve understanding of high hospital users and/or complex families.
Sharing information on trends/ emerging themes especially around issues such as why are children in hospital, reasons for admission, minor illnesses or child safety with partners and public health thereby identifying potential gaps in wider service provision.
Adhere to agreed Trusts’ policies and procedures. Work within the NMC Code of Conduct.
Will work flexibly across the Neo-Natal, Paediatric, Safeguarding and community services ensuring smooth communication.
To set safeguarding alerts on Systm1 for vulnerable children so that clinicians are alerted to the extra information, enabling them to deliver appropriate health care and aware of any safeguarding concerns.
Ensure care plans and information is documented clearly and concisely.
STAFF MANGEMENT
Will be responsible for the line management of the administrative support to the service.
Will be responsible for training other staff to enable cover to be provided for periods of absence.
Supervision of and facilitation of learning experiences for students from other disciplines as appropriate.
Use available resources to develop skills and knowledge for effective decision making, discharge and care packages.
To supervise other staff as necessary in the delivery of routine duties.
Clinical Care
Initiate, evaluate and maintain a high standard of care, ensuring that it is evidence based and compliant with best practice.
Ensure that care is family centred, sensitive and responsive to the needs of the children and their families.
Actively contribute to Essence of care benchmarking and action planning.
To work closely with Matrons, Consultants, Ward Managers, Nurses in the Emergency Department setting and all members of inter professional teams in the hospital to share timely information.
To act as lead practitioner, and autonomously manage a workload of hospital attendance notifications and have the ability to assess, plan and evaluate and prioritise information to be shared with health visiting and school nursing teams.
Triage clinical notifications/reports and coordinate liaison or transfer of information to School Nursing and Health Visitor Service in a timely manner in accordance with current Trust Policy.
Communication
Display excellent communication and interpersonal skills, acting as a role model to the team.
Provide an effective, high quality communication system which maintains continuity and safeguards the needs of Children and Young People accessing care across the hospital and community.
Provide and receive highly complex and sensitive information and communicate this information appropriately, including issues on matters of child protection/safeguarding concerns in compliance with information governance.
Participate in meetings and working groups within the paediatric department as required.
Establish and maintain robust partnership working with user groups, utilising user views and experience to improve the local service.

Participate in the production, monitoring and updating of clinical practice guidelines and procedures.
To participate clinical audit to make informed decisions with regard to improving service delivery and actively seek and utilise service user views to promote service developments.

Person Specification
Education/ Qualifications
• RSCN/ RN (child)
• Health Visitor/ School Nursing degree
• MIPS/ENB 998 or equivalent
• Evidence of continued professional development in safeguarding children to level 3.
• Master’s degree or equivalent experience in specialist field
Knowledge
• Demonstrate experience of co-working within the multi-agency team.
• Understanding of the wider healthcare environment.
• National and local safeguarding policies
