You will be the lead clinical pharmacist within one of the Health and Well-being teams for one of the five localities in South Devon, ensuring the delivery of a high-quality clinical pharmacy service related to medicines optimisation across your locality. The role will include supporting intermediate care teams, long-term condition clinics, care homes, and inpatient beds located within the community hospitals.
This is an exciting opportunity to provide maternity leave cover in a well-established and innovative medicines optimisation team, working in an intermediate care setting with strong links to both acute and primary care multidisciplinary teams.
You will provide expert information about medicines and medicines management to a range of audiences including service users, carers, and wider trust staff, ensuring services in your locality are patient-centred and delivered in a manner that meets the needs of patients and other stakeholders.
Developing and supporting services to meet changing operational or clinical needs, this role is a key link between primary and secondary care.
Main duties of the job
1. Be the medicines optimisation lead within one of the five localities.
2. Provide a clinical pharmacy service to the locality Health and Well-being team, including intermediate care.
3. Lead and deliver a specialist pharmacy service by providing in-depth, patient-focused clinical medication reviews to ensure safe and effective care.
4. Provide highly specialised pharmaceutical advice in highly complex situations to healthcare professionals, care home staff, patients, and carers, visiting patients in various settings.
5. Promote the safe, effective, and high-quality, cost-effective use of medicines.
6. Provide a clinical pharmacy service to community hospital beds within the locality, working closely with local General Practitioners and attending ward rounds for poly-pharmacy medicine reviews.

With strong links to the acute team, locality pharmacists are embedded within the community Intermediate Care Teams, providing vital support on all aspects of medication to patients and healthcare staff. The locality team provides a clinical pharmacy service to the community hospitals at Dawlish, Totnes, Brixham, and Newton Abbot. Qualifications
* Registration with GPhC
* Postgraduate Clinical Pharmacy Diploma or equivalent clinical pharmacy experience
* Proven evidence of Continuing Professional Development
* Specialised training in clinical pharmacy
* Member of the Royal Pharmaceutical Society
* Non-medical prescriber
Experience
* Proven ability to manage and make change
* Understanding of, and respect for, the roles and responsibilities of other healthcare professionals
* Demonstrable leadership skills and ability

Knowledge
