Job summary We have an exciting opportunity for a registered professional to join the Learning DisabilityForensic Team as a band 6 practitioner-22.5 hours a week. The team provide a service Monday- Friday 9am -5pm, the successful candidatewill have the opportunity to discuss flexible working patternwithin these hours. You will be working within a small specialist team of nurses, health care support workers, Psychiatry and Psychology. The Learning Disability Forensic Team are based at County Hall, Glenfield, Leicester and provide support,clinical assessments,interventions and risk management forpeople with learning disabilities presenting with forensic needs. The service is provided to people living in the community of Leicester, Leicestershire and Rutland and additionally to patients who are currently ininpatient psychiatric settings supporting their discharge into the community and thereafter. This post is an excellent opportunity for a registered professional (professional qualification and registration) who has some experience of working with people with learning disabilities and complex high risk / forensic needs to develop and strengthen clinical skills and competencies and additionally work within a team who are highly passionate in relation to supporting people with learning disabilities and complex needs to live successfully within the community. You will need a full driving license and access to a vehicle insured to use for work purposes Main duties of the job The successful candidate will Hold a small caseload of patients with complex needsliving in the community, completing specialist forensicrisk assessment, risk intervention and oversight Provide clinical assessment, support, planning and oversight for patients of whom are currently in psychiatric units with forensic needs preparing for a safe discharge and ongoing monitoring. Actas lead professional and completion of required assessments and record keeping Working collaboratively with inpt teams, commissioners and other case managers Support the Community Learning DisabilityTeams where patients are presenting with potential forensic risks- through consultancy and advice. Offer training and advice to carers, providers and health and social care staff specifically in relation to meeting the individuals forensic needs. Provide specialist advice and support to people to enable them to remain in the community and prevent, if possible, in-patient admissions. Where required the service also provide direct interventions. Monitor/ support until there is sustainable evidence that the risks are manageable/ reduced, usually handing over to LD Community teams for longer term monitoring.
