Job summary We have an exciting opportunity for a Rehabilitation Assistant in the Wigan Borough Community Division. This is a permanent full time post (37.5 hours) to be working across Monday to Sunday. We are aiming to recruit suitably qualified, enthusiastic and engaged individuals to our new Intermediate Care at Home Team. The Team will provide rehabilitation for patients in their own place of residence, and other community settings. Joining up the services provided by the 'Home first' Team in addition to Community Therapy, whilst working closely and flexibly with social care colleagues and the existing Intermediate Care Therapy Team who deliver rehabilitation into our existing bedded units. The ideal candidate will have an open minded and flexible approach, have a 'can do ' attitude and be willing to come into a new initiative to support the development and establishment of the service - have excellent communication skills, Function as an effective member of the multi-disciplinary team and the integrated team within Health and Social Care and also the ability to comfortably work across role boundaries. It is essential for this role that you have the ability to independently travel across the borough to assess, formulate therapy plans and deliver therapy interventions for patients in their own place of residence.
