Abhi Rose are looking for an experienced, motivated and confident Care Coordinator to join our team supporting clients in their own homes within North Kirklees.
Duties and Responsibilities
1. Client Assessment and Care Planning
o Conduct a comprehensive assessment of each individual client's care needs.
o Develop individualised care plans in collaboration with health care professionals, the client and their families.
o Regularly review and update care plans to ensure that clients' changing care needs are met.
2. Care Coordination
o Coordinate and manage the scheduling of staff rotas, ensuring appropriate coverage for clients' visits.
o Ensure appropriate staffing levels and continuity of care at all times.
o Monitor and adjust the rota as needed to accommodate changes in clients' schedules or staffing availability.
o Provide ongoing support to both clients and staff.
o On-call duty (rota shared between other staff).
o Respond to any urgent or emergency situation as part of the business needs.
