Job summary The Ross Practice in Harlow has a fantastic opportunity for a care coordinator to join our team on a full time basis, for 37.5 hours per week. The role will focus on coordination of the patient's journey to meet their physical and/or mental health needs, working closely with the 3 practices in the primary care network (PCN). If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you. Main duties of the job provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients. ensurepatient health and care planning is timely, efficient, and patient-centred and thatall patients receive the best possible care and service. support patients preparing for clinical conversations with a primary care professional or following up clinical conversations they have had. become an important member of your PCN multidisciplinary team (MDT) identifying patients in need of proactive support ensuring the appropriate support is made available to them and their carers as well as any needs being addressed. work alongside a variety of different people working in the practices and across the PCN including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician associates, paramedics, health and wellbeing coaches, podiatrists, occupational therapists and clinical pharmacists, as well as the administrative team.
