Joining us in Cranbrook, B.C., you’ll have a pivotal role providing social work services to in-patients attached to the East Kootenay Regional Hospital. You will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families. Working collaboratively with other healthcare providers and the multidisciplinary team, you will offer direct psychosocial care, counseling, system navigation, advocacy, education, and linkage to relevant specialized complex care resources while facilitating client self-management.
Key Responsibilities:
* Provide urgent responses, psychiatric assessments, and treatment planning for individuals referred to psychiatric services.
* Offer supportive treatment for patients in both short-term and long-term care, helping them navigate complex psychological needs.
* Work collaboratively with a multidisciplinary team to ensure comprehensive care for patients.
* Support the emotional, mental, and social well-being of patients while assisting them in developing coping strategies and accessing necessary resources.
