The post holder will provide advice for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients being treated. The post holder will demonstrate critical thinking in the decision-making process. Under the supervision of a GP Partner, they will work collaboratively as part of the primary care team and provide advice to meet the needs of patients, and their relatives/carers, while at all times working within the agreed policies and procedures supporting the way care is provided. The post holder will be an experienced Nurse Practitioner who holds an appropriate, specific, advanced professional qualification which meets NMC requirements for high-level practice. Main duties of the job Develop, implement, and embed health promotion and well-being programmes Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients Implement and evaluate individual specialised treatment plans for chronic disease patients with GP supervision where required Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF Provide wound care (ulcer/Doppler etc.) to patients if required Undertake the collection of pathological specimens as appropriate and request pathology services as necessary Process and interpret pathology and other test results as required Maintain accurate clinical records in conjunction with extant legislation About us Molebridge practice is a relatively small practice of around 6000 patients across 2 sites; we are a close-knit and supportive team and are looking to recruit like-minded, caring individuals who have an interest in supporting their local population.
