To provide a high standard of individualised patient-centred care to patients with a progressing and life limiting illness. You will need experience in a wide range of core Occupational Therapy skills as well as high level clinical reasoning and communication skills. To hold clinical responsibility for autonomously managing a complex patient caseload, undertaking appropriate and comprehensive specialist OT assessments and interventions for patients with life limiting conditions. To work flexibly and be accountable for efficient time management and prioritisation across community, inpatient and wellbeing services offered to patients of Woking and Sam Beare Hospice. To work independently and as part of the wider Hospice team applying clinical skills in holistic assessment of needs, treatment planning, goal setting, intervention and evaluation (use of outcome measures); including individual and group orientated approaches. To demonstrate excellent communication skills and to be able to adapt communication style to the situation, with the ability to communicate sensitive and challenging information effectively and with empathy. To contribute to the evaluation and continued development of the Therapies Service in line with departmental and organisational objectives.
