The Directorate is pleased to offer an exciting secondment opportunity for an enthusiastic Clinical Practitioner to work as a member of the One Coventry Integrated Team to support the prehospital and consultant led pathways.
The successful applicant will be working as a Clinical Practitioner to reduce unnecessary hospital admissions and contribute to effective admission avoidance. The role incorporates the use of high-level health assessment skills and a willingness to work towards the non-medical prescribing qualification to support patients from a medical perspective with a wide variety of conditions.
You will be required to demonstrate critical thinking in the clinical decision-making process; identify gaps within the service and assist with service improvement and development in line with national and local strategies. You will be responsible for facilitating effective teamwork and providing mentorship, teaching, and support across the multidisciplinary team.
This role is a fixed term 12 month contract to cover maternity leave.
Main duties of the job
1. Initiate effective history taking, clinical assessment, diagnosis, and the implementation of a range of care interventions for patients.
2. Utilise highly specialist knowledge and clinical reasoning to plan and provide clinical intervention in consultation with other colleagues in the multidisciplinary team.
3. Signpost, refer and support access to health, social and third sector services to reduce inappropriate attendances and admissions at A&E.
4. Work with and through others to support staff in the process of change.
5. Effectively manage caseloads identifying when patients are ready for either discharge or care transfer to other service providers.
6. Triage referrals into the service and ensure care interventions are implemented from a variety of sources.
7. Assess risk of providing care within the community and implement relevant mitigations.
8. Participate in 7 day working rotas to ensure delivery of key services.
