An exciting opportunity has arisen for an experienced Band 6 Physiotherapist with proven knowledge, skills and experience of intermediate care, rehabilitation and admission avoidance.
We are looking for an experienced and well-organised individual with a passion for promoting independence and a home-first approach.
You will have the opportunity to work flexibly across the tier of services, which includes:
* 13 Residential Intermediate Care (IMC) Beds (Currently located at Elmhurst)
* Killelea House (36 Enhanced and residential IMC beds)
* Bury Rapid Community Response Service & Hospital at Home
Home-based Intermediate Care and Reablement.
You will be part of a large multidisciplinary team that includes Advanced Practitioners, Nurses, Physiotherapists, Occupational Therapists, Social Workers, Care Support Workers, and Administrators.
The Services operate a 7-day service, currently covering the hours of 8 am to 8 pm, including weekends; therefore, the ability to work shifts to cover these hours is a requirement of the role.
Main duties of the job
The Intermediate Tier prides itself on providing strengths-based assessments and interventions to the people of Bury, helping them regain or attain as much independence as possible in their homes.
Our patients are at the heart of everything we do! We are looking for someone committed to being part of a healthcare team that delivers excellent care and support to our patients. You will be responsible for your own caseload, performing patient therapy assessments, providing a diagnosis, developing, implementing, and evaluating individualised holistic treatment programmes, and delivering a high standard of evidence-based care.
You will work within and have support from a multi-disciplinary team, follow appropriate pathways of care, and liaise with and refer to other agencies as appropriate.
We seek someone hardworking and motivated with excellent clinical, communication, organisational and time management skills. A flexible approach to day-to-day tasks is essential to meet the demands of the service. You will also need to travel across the service footprint, when necessary, in line with service needs, so access to transport is essential.

Knowledge and Experience
* Previous experience across a broad range of junior rotations.
* Specialist knowledge in anatomy and physiology to underpin relevant clinical skills.
* Knowledge of areas of clinical governance including quality, audit and risk management.
* Basic understanding of computers and their applications.
* Experience of working within a team and independently.
* Experience of liaison with other professionals and partnership working.
* Advance clinical reasoning skills.
Education / Qualifications
* Degree in Physiotherapy or equivalent
* Evidence of CPD in the form of a detailed personal development portfolio.
* Member of the Chartered Society of Physiotherapy.
* Membership of relevant special interest group.
Skills and Abilities
* Excellent written and verbal communication skills
* Able to present information in a clear and logical manner
* Proven organisational skills including time management, effective delegation and prioritisation and the ability to complete a task.
* Ability to work with and motivate others within a team.
* Ability to work autonomously within a clinical area with senior support.
* To comprehend and work within the Trust’s and Council’s policies of data protection, equal opportunities and health and safety to meet the differing needs of patients.
